We do not have in-network status with Martin’s Point or Tricare but often are covered with pre-authorization. Please contact us to discuss further.

Please bring your insurance card to the initial appointment. Also, please let us know if you are the subscriber on the plan or if it is held by a spouse. If so, we will need their name and date of birth. Please check with your insurance company to find out your specific benefit for physical therapy, specifically whether you are subject to co-pays or deductible and whether or not you need a primary care referral in order for our services to be covered by insurance. If you are checking to see if our practice is in network with your insurance, please check both the group, Oceanside Physical Therapy, and the specific provider, Cristin Zaimes, as she holds the contract for the group. We have had misinformation because of a few of the new marketplace plans and want to clarify. If you are a Medicare patient we will need to know the amount of PT benefit that you have used this calendar year.
